Authorities identified 76-year-old Phyllas Louise Fone, from West Salem, Ohio, as the woman who lost her life following a three-vehicle wreck that also injured three other people on Friday afternoon near Madison.
As per the initial information, the fatal hit-and-run crash took place on I-65. The 56-year-old driver of the Dodge Journey minivan said that a white or silver vehicle suddenly tried to switch into his lane of travel and crashed into the right front tire of his minivan, disabling the Dodge. The white or silver vehicle continued down the road after the collision.
The 30-year-old Jeep Wrangler driver informed the officers that he saw the Dodge stopped on the interstate and quickly applied his brakes but failed to stop before hitting the minivan from behind. According to the officials, 76-year-old Phyllas Louise Fone, who was in the back seat of a Dodge Journey minivan succumbed to her injuries after the accident.
Fone died just after arriving at Skyline Medical Center. The driver, front seat passenger of the minivan, and Jeep driver were taken to Skyline with non-fatal injuries. There was no sign of impairment on the part of either driver. Officers have not provided any information about the car that had struck the minivan. No other details are immediately available.
