The Department of Public Safety reported a motor vehicle accident in Phoenix on Saturday.
The officials reported that 55-year-old Major Raines was declared dead at the scene and his passenger 49-year-old man was rushed to the hospital in critical condition.
A 33-year-old woman was also involved in the crash and is currently in the hospital. She suffered non-life-threatening injuries.
The crash occurred after an SUV rolled off the eastbound Interstate 10 southbound Interstate 17 ramp.
DPS revealed that the woman will be facing charges when she will be released.
The investigation relating to the fatal crash is ongoing and active.
